Mid Valley Physicians Association (MVPA) is a non-profit Physician-Hospital Organization located at 1401 East 8th Street, Weslaco, TX 78596. This policy describes how information submitted through this website is handled.

What this website collects

The contact and membership inquiry forms collect the information you enter — such as your name, email address, phone number, practice name, specialty, NPI number, and message. Submitting a form is voluntary.

Like most websites, our hosting infrastructure keeps standard technical logs (such as IP addresses) for security and reliability. Form submissions are also rate-limited by IP address to prevent abuse.

How submissions are used

Form submissions are delivered to MVPA staff so we can respond to your inquiry. Submissions are processed and stored by JotForm, the form service this website uses, and are not used for marketing or sold to anyone.

What this website is not for

This website is an informational site for physicians and the public. Please do not submit patient health information through these forms. If you need to reach a physician's office about care, contact that office directly using the phone numbers in our directory.
